7:30 AM to 11:30 Am and 1:00 PM to 4:30 PM
Dr. Mitchell is out of the office Tuesdays and alternate Fridays.

NEW PATIENT REGISTRATION

Please bring a photo ID/ driver’s license for registration at your first visit.

PRESCRIPTION REFILLS

Please have your pharmacy contact our office between 7:30 AM and 11:30 AM at (805) 497-7529 for refills. We do not refill prescriptions when the office is closed as access to your medical records is not available outside of office hours.

EMERGENCIES

During the week, please call (805) 497-7529 and our answering service will reach the doctor on call after hours.

Weekends, we ask you to contact your local emergency room or urgent care for emergency medical care.

PAYMENT FOR SERVICES/ INSURANCE BILLING

Payment is expected at the time services are rendered unless prior arrangements have been made. If we are a member of your insurance plan, we will submit a claim on your behalf with payment due according to the appropriate provider contract. A current insurance card must be presented prior to services being rendered if you wish to have your insurance billed. Any office visit or surgical copay is due at time of service.

Please be aware that the filing of insurance claims does not guarantee payment by your carrier. Frequently, proof of eligibility, covered and non-covered services and various exclusions cannot be determined until your carrier processes the claim. Therefore, we do submit insurance claims with the understanding that you are responsible for any non-covered services as well as co-payments and deductibles.

Payment of the estimated patient responsibility for surgical procedures may be requested on the date of the procedure for cash and commercially insured patients.

We request that all patients provide a credit card on file to expedite processing of copays and patient responsibility portions of services provided after insurance coverage. If this is not acceptable to you, our billing department is available to discuss alternatives.

MEDICAL RECORDS RELEASE POLICY

We are pleased to comply with all requests to release copies of your medical records once a signed medical records release form is received. This form is available from the front desk. A flat $15.00 fee will be charged for administrative time and materials involved in copying and delivering the record. Additional fees may be incurred due to postal expenses.
